I, the undersigned hereby confirm that I am voluntarily engaging in an acceptable level of exercise and have sought where appropriate approval from my GP to exercise. And as such am aware of the associated potential risks involved. I hereby state that I have received, read, understood and answered honestly the PAR Q questionnaire relating to my health and read and understood the on-line induction presentation sent to me. I am aware that I may participate in activities including aerobic exercise, resistance and weight training and stretching. I realise that my participation in such activities involves risk of injury and even the possibility of death. I take full responsibility for any injury, illness or death caused by my improper use of any fitness equipment within this training facility. If I am unsure of how to perform any exercise or activity safely and effectively, I will seek professional guidance from a qualified member of staff employed by The John Pounds Centre Fitness Suite. I will not use any equipment within this facility unless I have received the guidance to do so. The health and safety of our team and customers is always our number one priority. If I am feeling unwell, experiencing or displaying any symptoms of illness, I will not attend the centre until I have recovered and/ or after consultation with my health professional.
